Nurse Practitioner

Under the supervision of the Medical Director, uses advanced clinical skills to assess, diagnose and manage acute and chronic health conditions for the patients of Partnership Community Health Center (PCHC).

 

MINIMUM JOB REQUIREMENTS: 

  • Masters Degree in Advanced Practice Nursing from an accredited school.
  • Current Wisconsin Nurse Practitioner licensure and licensure as an Advanced Practice Nurse Prescriber.
  • National Certification as a Nurse Practitioner.
  • Maintain BLS Certification

 KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED: 

·         Demonstrate the PCHC mission by providing compassionate, culturally sensitive, quality health care to low-income patients.

·         Ability to work well with a diverse population, including staff, providers, patients and members of the public on a regular basis.

·         Communicates issues effectively to the Medical Director and/or collaborating physician.  Notifies the appropriate physician of any issues that may need immediate attention or intervention.

·         Performs procedures for diagnostic and/or therapeutic purposes.

·         Knowledge of outpatient clinical administrative procedures.

·         Ability to maintain quality, safety, and /or infection control standards.

·         Orders, conducts and interprets laboratory tests and diagnostic studies.


DUTIES AND RESPOSIBILITIES:

 

  1. Provide direct ambulatory care nurse services, including patient history, examinations, evaluation, treatment and teaching in accordance with the Provider Expectations Policy.
  2. Coordination and supervision of selected subspecialty areas.  Liaison with subspecialists to include patient problems, lab and triage.
  3. Review of labs and test results in a timely, efficient manner.
  4. Participate in quality improvement and compliance activities as required.
  5. Participate in PCHC’s on call schedule as determined by the Medical Director. 
  6. May participate in clinical outreach activities as assigned.
  7. Practice safety, environmental and/or infection control methods.
  8. Assist in development of PCHC’s annual health care plan regarding issues specific to health center target population.
  9. Perform other duties as assigned.
