Case Manager - Shelter

Stephen Center has served homeless and low-income individuals in Omaha since 1984 with the mission to partner with the community, families and individuals to overcome homelessness, addiction and poverty.  The Pettigrew Emergency Shelter and Permanent Supportive Housing offerings provide safe and secure housing while individuals and families complete an assessment of permanent housing options. All Stephen Center residents have access to dual diagnosis support services via the HERO addiction/recovery program. 100% of Stephen Center shelter supportive housing participants are living in poverty.  Housing transition support identifies poverty barriers that may impact one’s ability to achieve or maintain permanent housing through case management, life skills and behavioral health services. Stephen Center serves approximately 250 people daily in all programs.  

Day position: Monday thru Friday                                                                                                                                                                                          Starting Pay: $17.50/hour more for Bilingual

Purpose: The case manager will assess each Stephen Center client to determine the need for physical and mental health services, housing and employment assistance, legal advice, services for children, or other support as required.  The case manager will then work closely with the Shelter Director and Case Manager Supervisor and appropriate community agencies and services to provide the best possible solutions to each client’s challenges.  Documentation of all client assessments, services provided, and outcomes will be essential to acceptable job performance.

Key Accountabilities:

Prepares individual service plans for each admission within 72 hours of intake, consulting with the managers and others as needed.

Follows through with the implementation of the service plan, working with the client and the appropriate community services and agencies.  Formally documents all actions that are taken, client involvement, outcomes, problems, etc.

Meets with the shelter director and case manager supervisor to consult on client service plans and to keep them informed regarding client status.

Manages the filing of all service plan records generated by coordinating the development of a functional record-keeping system to serve each client as well as Stephen Center staff information and statistical reporting needs.

Enters data into Clarity within 48 hours of service.


When a service plan is successfully completed or the client leaves the Stephen Center, finalizes the service plan record and files for future reference.

Develops an understanding of the roles and challenges of other Stephen Center staff members and key volunteers and the daily operation of the Shelter in order to work effectively within that framework.

Documentation of all client assessments, services provided, and outcomes will be essential to acceptable job performance.

Position Requirements:

 A 4-year degree in a related field is required or a combination of a 2-year degree in a related field and related experience.  

Proficiency in Microsoft Office applications.

Proficiency in Spanish, a plus

Requires the ability to become assertive in the enforcement of organizational rules, regulations and policies of Stephen Center.

Ability to two way verbally communicate

Conducts self in a manner of the professionalism that is conducive to recovery from mental illness, co-occurring disorders and overall wellness, including a minimum of one year of continuous sobriety if in recovery, from the substance for which you are in recovery.
