Care Management Registered Nurse
The Care Management Register Nurse (CMRN) provides RN Care
Management designed to improve individual and/or family overall health and
well-being to a specified/targeted patient group. The CMRN will conduct an
initial assessment to identify actual needs and potential risks for adverse
outcomes. This position will coordinate comprehensive health care in
collaboration with, but not limited to health care providers, community service,
agencies, and schools as appropriate to meet the needs and risks
identified. The CMRN will develop,
implement, coordinate and evaluate patient care plans in an outpatient
environment.
MINIMUM JOB REQUIREMNTS:

Must
be a licensed registered nurse in the State of Wisconsin, Baccalaureate in
Nursing preferred.

Care
management certification preferred, three to five year experience preferred.
DUTIES AND
RESPONSIBILITIES:
1. Conduct
initial and ongoing assessments of each patient with respect to clinical
condition, areas of need/risk, level of care and plan of care.
2. Apply
decision support criteria appropriately according to evidence based clinical
practice guidelines.
3. Partner
with, and advocate for, patients and/or families to assess, develop, implement,
and monitor actions to improve health status and quality of life resulting in
comprehensive, cost-effective care plans, using a multidisciplinary process
documented in the electronic health record Epic.
4. Facilitate
a learning environment with patient and families to address self-management
and/or educational needs.
5. Coordinate
other care team members such as physicians, nurse practitioners, nurses, care
navigators, community service agencies, etc. to promote quality care/education.
6. Work
with payors and manage payor portals and patient lists to improve patient health
outcomes within payor groups.
7. Participate
in data collection and research related to patient level outcomes and
community-based activities, as appropriate.
8. Identify
and report quality and risk management issues to the QI Coordinator and Risk
Manager.
9. Assist
in development, revision and implementation of care management policies and
procedures.
10. Participate
in staff meetings, self-directed work teams, and other program initiatives
related to performance improvement.
